2016 Influenza Shot Clinics
Community Health Services will again be providing Influenza (flu) shots for our students. Please read the DOH Seasonal Influenza Parent Letter 2016 as there are important changes from last year. Print and complete the consent form along with the vaccine information statement. Every child receiving a flu shot will need a copy of their insurance or Medicaid card attached to the consent form. A parent/guardian of students Kindergarten – 5th grade will need to be present while the vaccine is being administered. The scheduled date and time for each school can be found under the School Flu Shot Schedule 2016 link. All paperwork should be brought to the shot clinic. Please do not turn your forms into the school. Your child does not have to attend their home school shot clinic. They may attend whichever school works best for your schedule.

Head Lice Policy
The parent of a student found to have live head lice will receive a personal call or a note sent home regarding treatment. A student with live lice may be excluded immediately in the event of frequent or repeated infestations or at the discretion of the school nurse.
